I understand that my access to Giving Account information is for the express purpose of providing service to my Firm’s clients who are also Fidelity Charitable Account Holders, and who have authorized me or the Firm with the appropriate permissions.
The Firm and⁄or I will notify Fidelity Charitable of personnel changes that affect access options and levels.
I understand that this access does not allow me or the Firm to deduct fees from any Giving Account and that Fidelity Charitable provides this access to me and/or the Firm exclusively at the Account Holders written request. Such access can be revoked by either the Account Holders or Fidelity Charitable at any time, without prior written acknowledgement to me or the Advisor Firm.
Fidelity Charitable will provide to me instructions for accessing Giving Account information online and/or via phone. I understand that it is my responsibility to monitor access to my Username by representatives of the Firm to ensure its proper and legal use. I understand that, for security purposes, Fidelity Charitable has the right to require periodic password resets and to issue new Usernames.
I understand that reported Giving Account balances represent irrevocable and nonrefundable contributions to Fidelity Charitable, which are exclusively owned and controlled by the Fidelity Charitable Trustees. I understand that they are not and do not represent assets of the Account Holder.
Under policies of Fidelity Charitable, and in accordance with the anti-money laundering regulations applicable to the various financial institutions that provide financial services to Fidelity Charitable, I understand that Fidelity Charitable may obtain, record, and verify information that identifies each person who establishes a Giving Account and other people who contribute, or have access, to the Giving Account. I understand that Fidelity Charitable may ask for the name, address, Social Security number, date of birth, and other information that will allow it to identify people with access to a Giving Account, and may ask to see individual driver’s licenses or other identifying documents and may verify the information obtained.
